If you feel you need to have your loved one assessed at the Neuropsych Clinic, you will need to have a referral from your GP, NDIS Support Co-ordinator or Psychologist.
If you are unable to obtain a referral from a medical professional or allied health member, you can give us a call and we can assess your eligibility.
If you are a GP, Medical Specialist, Allied Health Member or Support Co-ordinator, please use the following form to contact us, or email tarah@neuropsych.clinic
We provide:
assessment of memory and cognitive functioning and cognitive decline
diagnosis and management of neurocognitive disorders, including dementia and related behavioural disturbances
functional assessment
psychoeducation and links to community resources
family and carer psychoeducation and psychological therapeutic services
support worker training
assistance in future planning
As a part of the referral please include:
History of the cognitive difficulties
Any recent medical reports such as hospital discharge summaries, brain scan reports or other medical investigation reports (e.g. E/LFT, B12, FBC, TSH, Vitamin D)
A list of medications the client is currently taking
A list of any other relevant medical conditions
On the day of the appointment, both the client and the caregiver will be working with Tarah, a neuropsychologist. An extensive clinical interview will be taken which will inform the cognitive assessment.
We use both a psychometric and qualitative approach, rather than full assessment batteries. This ensures the assessment is targeted and individualised to the needs to the client. When the assessment of the client commences, a further caregiver burden and stress interview will commence. This gives caregivers a further opportunity to discuss their concerns and needs so that supports can be put in place.
The appointment time allows for frequent breaks if needed, in a quiet and inviting space. Once the assessments are complete, initial feedback will be given and a follow up appointment time will be allocated.
The follow up appointment will allow for extensive feedback and treatment plan discussion.
